Hospital’s Home Health and Hospice Programs Highlight a Year of Exceptional Care
Columbus Community Hospital’s (CCH) home health and hospice programs both offer skilled services to community members in the comfort of their own homes. Home health provides health care for patients who are recovering from surgeries or illnesses, or those who have chronic health conditions. Hospice is for patients with terminal illnesses who no longer benefit from curative treatment, and want to be comfortable in their final stages of life.
“Meeting people where they are in life is our focus in both home health and hospice,” said Jody Evans, RN, BSN, the programs’ director. “For some, this is assisting patients after they have moved into our service area to be closer to family, and discover they need help finding local health care. For others, this is caring for them in the end stages of life.”
The hospital's home health and hospice programs continue to grow and evolve to provide high-quality care for the surrounding communities they serve. The 2023-24 home health and hospice annual report showcases both of these programs’ accomplishments and growth.
Some highlights in the annual report include:
Staff certifications: Several caregivers have grown and expanded their skill set, and acquired specialized certifications to continually improve patient care.
Data showing patient-centered care: The National Research Corporation shared recent statistics of reviews and ratings from patients online.
Patient testimonials: Patients shared their positive experiences with the programs.
The annual report focuses on the fiscal year 2023-24, between May 1, 2023 and April 30, 2024.
