Prairie Lakes Healthcare System is Hiring - Social Worker – MSW/BSW - Home Health/Hospice - Dialysis

Primary focus will be Dialysis, Home Health and Hospice patients. Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem. Facilitates financial assistance for patients. Formulates social service care plans to meet complex psychosocial needs for patients. Provides interventions to assist patient in optimum social and health adjustment within patient's capacity.
