Cardiopulmonary Rehabilitation Department Recognized As Age-Friendly

All older adults deserve safe, high-quality health care based on what matters most to them.

Columbus Community Hospital is proud to announce the cardiopulmonary rehabilitation department has earned the designation of an Age-Friendly Health System — Committed to Care Excellence. This honor recognizes that the hospital is leading the way to ensure every older adult receives age-friendly health care.

“The number of individuals over 65 continues to grow rapidly. As we age, care becomes more complex and involves challenges,” said Jen Fjell, director of cardiopulmonary & the W.O.C. Health Center. “The cardiopulmonary rehabilitation department is excited to help provide the best possible outcomes in the patient’s journey to recovery.”

As part of the Age-Friendly Health Systems movement, CCH is among the first health systems in the country to implement age-friendly health care. The initiative focuses on a series of practices based on addressing four essential elements of care for older patients:

  • What Matters: Know and align care with each older adult’s specific health outcome goals and care preferences.

  • Medication: If medication is necessary, use Age-Friendly medicines that do not interfere with What Matters to the older adult, Mobility, or Mentation across care settings.

  • Mentation: Prevent, identify, treat and manage dementia, depression and delirium across care settings.

  • Mobility: Ensure that older adults move safely daily to maintain function and do what matters.

“By being the first medical facility in the area to join the Age-Friendly Health Systems movement, Columbus Community Hospital has shown that it is committed to the health care of our older adult population,” said Fjell. “This commitment to excellence will continue to spread as we encourage all areas of CCH and the community medical facilities to join this growing initiative and improve health care. We hope the 4M’s will guide every encounter with older patients and their families to create a care plan that focuses on the individual’s wellness and strength.”
