Sanford Health’s New Remote Patient Monitoring Service Launches In Sioux Falls

Sanford Health is offering remote monitoring to help patients stay healthy from the comfort of their home.

Sanford CareSignal allows patients to stay in contact with their care team between appointments. This new service lets the patient self-report symptoms either via phone call or text message and helps reduce avoidable hospital admissions. There is no cost to enroll for eligible participants. Interested patients will be enrolled by their clinician. Once enrolled, CareSignal will reach out with a phone call to help the patient opt in and participate in the program.

Sanford CareSignal helps monitor:

  • Chronic obstructive pulmonary disease (COPD)

  • Depression

  • Type 2 Diabetes

  • Heart failure

  • Recovery after discharge from the hospital or emergency department

“We are excited to roll out this new program to patients in the Sioux Falls region,” said Lindsay Daniels, Sanford Health system executive director, care management. “We want our patients to have access to health care whenever they need it. This will help patients self-manage their health care and be confident in the decisions they are making as it pertains to their health.”

Once enrolled in the program, patients will receive automated phone calls or text messages to check in and monitor their health. If they respond that symptoms are not improving, a care team member will follow up with additional support. Participating in Sanford CareSignal only takes a few minutes each week, and participants can choose a time to be contacted.
