Community Health Needs Assessment Survey Now Available

Siouxland District Health Department, MercyOne and UnityPoint Health have joined forces to launch the 2024 Community Health Assessment (CHA), which is completed every three years and aims to gather valuable insights into the health needs and priorities of Siouxland residents.

One component of the CHA is a community-wide survey, which will be conducted over the next month. It seeks to engage community members in a dialogue about their health concerns, experiences and goals. By understanding our community's unique challenges and opportunities, we can better tailor our programs and services to address the most pressing health issues.

The survey covers topics such as access to care, community health strengths and barriers, social determinants of health and more. It will be available online at https://siouxlandhealth.qualtrics.com/jfe/form/SV_eKYujg8ysFx5m98  and distributed through various community channels to reach as many residents as possible. The survey is also available in multiple languages.

This assessment is a valuable public health tool for informing community members and decision-makers about community health issues and strategies.

The survey results will be carefully analyzed to inform the development of a CHA Report, which will be available to the public later this fall. This report will serve as a roadmap for prioritizing health initiatives and allocating resources effectively through a health improvement plan.
